I understand a student must be at least 15 years of age by the first day of class to attend driving training education courses

I am willing to give the necessary time and effort in order to fulfill the requirements of this approved course as prescribed by the Washington State Department of licensing. I understand that I will receive 30 hours of classroom instruction, 6 hours of behind-the-wheel (btw) instruction, and at least 1 hour of btw observation during the course. This course will be completed in no-less than 4 consecutive weeks and in no-more than 16 consecutive weeks.

The course fee is $530 ($265 Deposit Required that is applied to course fee); fee covers 30 hours of classroom instruction and 6-1-hour student drives, as well as 3 attempts at the state required knowledge exam and 2 attempts at the state required skills exam in the Integrity vehicle. Payment is necessary to reserve a seat in the class. All applications must be received before the end of the 3rd day of class. All payments, fees and fines must be paid before the student will be allowed to take the written final and/or drive #6 (final drive). Required state testing must take place within 45 days of student course completion (drive 6) or all testing will be charged at regular price. I.D.S will determine when classes are held and which school and times. Please make checks payable to “Integrity Driving School.” Returned checks will require a fee of $40.00

Within ten days of the course start date an Online Permit Application is available to those students who have pre-registered. The Permit Application will allow you to obtain your instruction permit prior to the course without having to take the knowledge exam. It is required for anyone 15 – 15 ½. You must return this completed registration form and your payment to Integrity Driving School at 2640 E Edison Avenue in Sunnyside, WA, or complete and submit online at Integrity-driving.com in order to obtain the Permit Application form. I understand that I must have my instruction permit no later than the first drive lesson.

I understand that I must show the instructor my Instruction Permit at every behind-the-wheel (btw) lesson. To cancel a pre-arranged drive lesson, I must notify the Instructor at least 24 hours in advance –OR– if my drive is scheduled on a weekend, I must notify Integrity Driving School by Friday at 3 PM to reschedule or ask questions about my schedule due to the office being closed on weekends. If I do not bring my driving permit, do not bring my required glasses/ contacts, do not appear for a drive (and/or not be more than 10 minutes late for drive), or do not notify the instructor of cancellation in a timely fashion, I will pay a $50.00 no-show fee before the next drive. Students requiring additional driving lessons beyond 6 hours will need to pay $50.00 fee for each additional lesson.

I understand that outside driving practice is a necessary component to proceed satisfactorily through the course. I am also required to drive at least 10 additional hours outside of class with a qualified adult who has insurance coverage on his/her personal vehicle. To provide guidance and tools to meet this requirement, the first class will be a meeting at which the student and at least one parent/guardian attend.

Video / Audio / Photo Agreement

Integrity Driving School strives to teach students to drive with values, habits and morals above the average driver. With such integrity a driver will be better equipped to handle most any situation requiring a sudden response to ensure safety for themselves and those around them.

To prepare new drivers we offer video recordings in class, and in the car. Photographs are taken for visual effects that assist instructors teaching in class. We continue to improve using greater tools to create a stronger learning environment.

Your signature of permission includes full use of all photographs and videos taken by Integrity Driving School belong only to Integrity Driving School, LLC and for their sole use. These will be used for all purposes including all advertising and online use such as Facebook and I.D.S. website.

At the request of the parent a digital file will be created and given to them for a $5.00 fee.

Knowledge and Skills Exam

The fee for the Knowledge exam is $30.00 each exam; IDS student 1st 3 attempts included.
The fee for each Skills exam is $50.00. Additional $20.00 using the Schools Training car. IDS student’s 1st 2 attempts included (if within 45 days of course completion).
Arriving more than 5 min late creates a delay for others after you, therefore your appt will need rescheduled.
The Knowledge exam consists of 40 multiple-choice questions about traffic safety laws. Applicants must answer at least 32 questions correctly to pass the exam.
The drive test will test the applicants’ ability to drive legally and safely. Applicants must pass the driving test with a score of least 80%.
Your exam scores will be entered into the DOL database within 24 hours. You will have to visit the DOL office to apply for your license. Passing the knowledge and drive test does not guarantee that an applicant will be issued a driver license. The Department of Licensing will determine if an applicant has met all licensing requirements. If you are under the age of 18 you must be at least 16 years old, successfully completed a traffic safety education course, have had your permit for at least 6 months and have had 50 hours driving experience with at least 10-night driving hours to apply for your driver’s license.
The Department of Licensing reserves the right to conduct random re-examinations; applicants refusing to take a reexamination may have their license revoked.
Refunds will be handled as follows: There are no refunds on any knowledge or skill exam.
Rescheduling will be handled as follows: Rescheduling must be done with at least 24 hours' notice.

We reserve the right to refuse services to anyone.
If you received a letter from DOL to take an examination due to a physical, mental, visual or any medical issue, you must test at a DOL office. Failure to disclose this information prior to testing will result in your test not being honored at DOL to obtain your license and we will not refund the test fee.
Your car must be legal to operate on public streets. This includes, but is not limited to windshield, seat belts, wipers, defroster, tires, mirrors, all brake and reverse lights, turn signals and be legally licensed/tabs current.
You will need to demonstrate arm signals prior to beginning the skills test.
Pursuant to RCW 46.20.720 a driver convicted of an alcohol offense may be required to have an Ignition Interlock Device (IID) in order to drive. The applicant must take the skills exam in their vehicle using the IID.
Applicants wishing to use their own vehicle for their skills exam are required to provide proof of insurance and current registration, (current tabs), on the day of the test.
At the time of the test, provide proof of identification such as a photo permit, out of state license, or school yearbook.
Applicants who fail to arrive within 5 min. of scheduled drive appointment or reschedule with less than 24-hour notice, are subject to pay a $25 reschedule fee.
Applicants who fail to show up to drive appointment twice, consecutively, are subject to repay for drive appointment again.
All drives in Integrity vehicles are recorded.
